CONTRACTUAL AGREEMENT 

Because Fitness by Reese is committed helping you achieve your desired results, the following terms and stipulations have been included in this agreement:

I understand that I am responsible for all automatic draft payments, which occur on the 7th of each month.  In the event that automatic draft discontinues on the account noted above, I must provide a new and active account for the remaining payments, or settle the remaining balance in full.

 Any returned or failed payments will be considered delinquent.  A 5-day grace period is allowed for delinquent accounts, after which the collections process shall begin.  Any accounts considered delinquent for 25 days are eligible to be sent to collections.  Accounts ultimately sent to collections are remitted for the remaining payments and automatically grossed up 30% to cover collection costs.

 1.        I understand that should I quit the program for any reason other than medical, I will not be refunded any money and will still be responsible for paying the remaining balance of this contract.  Allowable medical conditions should be documented in the form of a detailed and signed Doctors note.

2.        Furthermore, I understand that my individual progress and success during this program depends entirely upon my efforts during my sessions, assignments I am required to complete on my own regarding this program, following the nutritional guidance provided by Fitness by Reese as well as following the general stipulations outlined above in this contract.

3.        I agree that my trainer is not liable should I suffer any injury during the course of this program. 

4.     I fully understand the terms of this agreement and what services I am entitled to with this program.
